How to Get Healthcare Forms
The State of California has a healthcare mandate, which means that we require all clients who live in CA provide proof of health insurance with their tax documents. We need proof for every person in your household - taxpayer, spouse & all dependents. If you did not get your forms in the mail, please see below on how you can get your forms.

Medi-Cal Coverage
Form 1095-B
You can get this by calling

Employer Coverage
Form 1095-C
Please contact your employer and ask for a completed 1095-C. This will include the names of all persons covered by your plan, and will indicate which months they were covered by your insurance.
